TeamSync: Stroke Team Simulation for Transitions of Care

TeamSync: Stroke Team Simulation for Transitions of Care is a virtual simulation in which teams of advanced-level students from different health professions work together to coordinate care and develop a discharge plan for a patient who has experienced a stroke. This simulation is led by the HIPE Center and was developed by a task force representing 5 professions with consultation from an expert panel representing 9 professions. 

TeamSync is held during the fall and spring terms each academic year. Learner self-assessment data indicates that outcomes are being met across assessment domains. This capstone experience is the most advanced level of the HIPE Center’s Health IPE Microcredential Program and participants will earn the Transitions of Care microcredential upon completion of all requirements.
